Discharge from hospital can be planned or advised
at short notice ahead of a weekend. Hospitals are
keen to fast track discharge for various reasons.
Returning to your home without additional support
is less than ideal at a time when you want to recuperate
and regain your independence. |

HOW
CAN WE ASSIST?

Meet you at the hospital
ahead of discharge and take instructions
for preparing your residence, shopping for
food supplies and running errands |
|
Accompany you from
hospital on your way back home |
|
Assist you in the early
hours / days post discharge by extending
trusted companionship, provide personal
care, liaise with family / Doctor / Pharmacy
as well as community nurses, physiotherapists
and other allied health professionals. |

We are able to put
in place nursing oversight either for set
time/date or on as on call. |
|
Typically, a single
Companion will support you to regain independence
within framework of a restorative care model. |
|
Your companion will
be only too happy to function as an extended
member of your household or a trusted friend.
Your companion will do as much or as little
as you require. Companions can accompany
you to outpatient appointments, take you
around to see friends or run errands on
your behalf. |
